Why Aquarium Fish Get Sick

Disease in aquarium fish is almost never random. The overwhelming majority of infections — bacterial, parasitic, and fungal — are opportunistic: the pathogens are already present in virtually every aquarium, held in check by the fish's immune system. Disease breaks out when something compromises that immune defence. Understanding the root causes is the single most important step toward keeping a disease-free tank.

The primary triggers are:

  • Poor water quality: Elevated ammonia (above 0.25 ppm), nitrite (any detectable level), or nitrate (above 40 ppm in freshwater, 20 ppm in saltwater) directly suppresses immune function. Ammonia burns gill tissue, opening pathways for bacterial invasion.
  • Temperature instability: Swings of more than 2 °C within 24 hours stress fish significantly. Heater malfunctions are a leading cause of Ich outbreaks.
  • Overcrowding: High bioload degrades water quality faster than filtration can compensate, while social stress from territorial aggression raises cortisol levels chronically.

Disease Diagnosis Flowchart

Accurate diagnosis is critical — treating for the wrong disease wastes time, stresses fish with unnecessary chemicals, and allows the real problem to advance. Use this symptom-based approach:

Ich (White Spot Disease)

Ich Symptoms and Life Cycle

Ich is the most common disease in freshwater aquariums worldwide. The causative organism, Ichthyophthirius multifiliis, is an obligate parasite with a three-stage life cycle that dictates treatment strategy:

  1. Trophont stage: The visible white cyst embedded in the fish's skin, feeding on tissue. Medication cannot penetrate the cyst wall at this stage.
  2. Tomont stage: The mature parasite drops off the fish, encysts on the substrate or décor, and divides into up to 1,000 daughter cells (theronts) over 6–72 hours depending on temperature.
  3. Theront (free-swimming) stage: The only stage vulnerable to medication. Theronts must find a host within 48 hours or die.

Visible symptoms include small white spots (0.5–1.0 mm) scattered across the body and fins, clamped fins, flashing against objects, lethargy, and loss of appetite. Heavy infestations cause laboured breathing as the gills become coated.

Ich Treatment Protocol

Because only the free-swimming theront stage is vulnerable to treatment, you must sustain medication through the entire life cycle — typically 10–14 days at tropical temperatures:

  1. Raise temperature to 28–30 °C (if species-safe). This accelerates the parasite's life cycle, bringing theronts into the water column faster where medication can kill them.
  2. Dose a malachite green/formalin combination (e.g., Seachem ParaGuard, Kordon Ich-Attack) or copper-based medication (e.g., Seachem Cupramine at 0.5 mg/L). Follow manufacturer dosing precisely — half-doses for scaleless fish (loaches, catfish, stingrays).
  3. Add aquarium salt at 1–3 g/L as a supplementary osmotic stressor against the parasite. Not suitable for all species — avoid with Corydoras, tetras sensitive to salt.
  4. Maintain treatment for a full 14 days even if spots disappear after 3–4 days. Stopping early allows surviving tomonts to reinfest.
  5. Perform 25 % water changes before each re-dose to maintain water quality and remove free-floating tomonts.

Fin Rot and Tail Rot

Fin rot is a bacterial infection — usually Aeromonas, Pseudomonas, or Vibrio species — that progressively erodes fin tissue from the edges inward. In advanced cases, it reaches the fin base and invades the body, becoming life-threatening. Fin rot almost always indicates underlying water quality problems: it is rare in well-maintained systems.

Symptoms: Frayed, ragged fin edges, often with a white or red margin. As it progresses, large sections of fin dissolve, and the remaining tissue may develop a bloody or blackened appearance. Secondary fungal infection (white, cotton-like growth) frequently accompanies advanced fin rot.

Treatment protocol:

  1. Test and correct water parameters immediately. Perform a 50 % water change. Ensure ammonia and nitrite read 0 ppm, nitrate is below 20 ppm. In mild cases, pristine water alone halts progression and allows regrowth within 2–4 weeks.
  2. For moderate cases: Dose an antibacterial medication containing erythromycin (API E.M. Erythromycin) or kanamycin (Seachem KanaPlex). Treat in a hospital tank when possible to protect the biological filter.
  3. For severe cases with body involvement: Combine kanamycin with nitrofurazone (API Furan-2) for broad-spectrum coverage. Consider medicated food containing oxytetracycline for internal bacterial load.
  4. Add Indian almond leaves or StressGuard to promote mucus coat regeneration and reduce stress during recovery.

Velvet Disease (Piscinoodinium)

Velvet is caused by the dinoflagellate parasite Piscinoodinium pillulare (freshwater) or Amyloodinium ocellatum (saltwater). It is often more lethal than Ich because it is harder to spot in early stages — the parasites are smaller than Ich cysts, producing a fine golden or rust-coloured dusting rather than distinct white spots.

Symptoms: A fine gold, rust, or yellowish sheen visible on the body when light catches it at certain angles. Fish clamp fins, scratch against objects, breathe rapidly, and become lethargic. Velvet attacks the gills aggressively, and respiratory distress is often the first noticeable sign before body dusting becomes visible.

Treatment: Copper-based medications are the gold standard — Seachem Cupramine dosed to 0.5 mg/L and maintained for 14 days. Dim the lights or black out the tank, as Piscinoodinium contains chlorophyll and relies on photosynthesis during its free-swimming stage. Raise temperature to 28–30 °C to accelerate the life cycle. Velvet is highly contagious — treat the entire display tank, not just affected individuals.

Columnaris (Cotton Wool Disease)

Columnaris, caused by the bacterium Flavobacterium columnare, is frequently misdiagnosed as a fungal infection due to its white, cotton-like appearance. Unlike true fungus, columnaris patches are more mat-like and greyish-white rather than fluffy. It can present as mouth rot, saddle-back lesions (a pale band behind the dorsal fin), or gill necrosis.

Key differentiator: True fungus (Saprolegnia) almost always appears on damaged tissue — wounds, missing scales, dead eggs. Columnaris can appear on apparently healthy skin and spreads rapidly at temperatures above 25 °C.

Treatment: Lower the temperature to 22–24 °C (if species-safe) to slow bacterial reproduction. Dose kanamycin (Seachem KanaPlex) or a combination of nitrofurazone and kanamycin for severe cases. External salt baths (10 g/L for 10–30 minutes) can reduce bacterial load on the body surface. Columnaris thrives in hard, alkaline water — if your GH is above 15 dGH, consider a gradual reduction during treatment.

Dropsy: Causes and Emergency Response

Dropsy is not a disease but a symptom — the visible result of fluid accumulation in the body cavity, causing the abdomen to swell and the scales to protrude outward in a characteristic "pinecone" pattern when viewed from above. By the time this symptom is visible, internal organ damage (usually kidney failure) is advanced, and the prognosis is poor.

Underlying causes include: chronic bacterial infection (Aeromonas hydrophila is the most common culprit), viral infection, liver or kidney failure from long-term poor water quality, and internal parasites. Dropsy is not contagious per se, but the conditions that caused it may affect other fish.

Emergency response:

  1. Isolate the affected fish in a hospital tank immediately.
  2. Add Epsom salt (magnesium sulphate) at 1–3 g/L to help draw fluid from the body cavity via osmosis.
  3. Dose a broad-spectrum antibiotic: kanamycin (Seachem KanaPlex) combined with metronidazole (Seachem MetroPlex) covers both aerobic bacteria and anaerobic/protozoan infections.
  4. Offer medicated food containing kanamycin or oxytetracycline if the fish is still eating.
  5. Monitor closely for 7–10 days. If scales return to a normal position and swelling subsides, continue treatment for a full 14-day course. If there is no improvement after 5–7 days, the prognosis is very poor.

Swim Bladder Disorder

Swim bladder disorder manifests as loss of buoyancy control: affected fish float at the surface, sink to the bottom, swim at odd angles, or flip upside down. It is especially common in fancy goldfish, bettas, and balloon-morph cichlids — breeds with compressed body shapes that predispose them to swim bladder dysfunction.

Common causes:

  • Overfeeding / constipation: The most frequent cause, particularly with dry foods that expand in the gut, pressing against the swim bladder.
  • Bacterial infection: Inflammation of the swim bladder from systemic infection.
  • Physical injury: Trauma from aggression, rough netting, or shipping stress.
  • Congenital defect: Common in heavily inbred fancy goldfish and balloon morphs.

Treatment: Fast the fish for 48–72 hours, then offer a skinned, blanched pea (the fibre acts as a gentle laxative). If constipation is the cause, improvement is usually visible within 24–48 hours of the pea feeding. For bacterial swim bladder infections, dose kanamycin or erythromycin in a hospital tank. For congenital cases, management (feeding sinking foods, lowering water level to reduce swimming effort) is the only option.

External and Internal Parasites

Beyond Ich and Velvet, aquarium fish face a range of parasitic threats:

  • Gill flukes (Dactylogyrus) and skin flukes (Gyrodactylus): Microscopic flatworms that attach to gills or skin. Symptoms: flashing, laboured breathing, excess mucus production. Treat with praziquantel (API General Cure, Hikari PraziPro) at 2.5 mg/L.
  • Anchor worms (Lernaea): Visible crustacean parasites that embed into flesh. Remove manually with tweezers, dab the wound with iodine, and treat the tank with diflubenzuron or potassium permanganate to kill larval stages.
  • Fish lice (Argulus): Flat, disc-shaped crustaceans visible to the naked eye. Remove manually and treat with diflubenzuron.
  • Internal worms (nematodes, cestodes): Indicated by white, stringy faeces, wasting, bloated abdomen. Treat with levamisole (in-tank at 2 mg/L) or fenbendazole-medicated food.
  • Hexamita / Hole-in-the-Head: A flagellate protozoan causing pitting erosion on the head and lateral line, especially in cichlids and Discus. Treat with metronidazole at 250 mg per 40 litres, combined with improved water quality and diet.

Disease Prevention: The 5 Pillars

Treating disease is stressful for fish and keeper alike. Prevention is always the better path. These five pillars form the foundation of a disease-free aquarium:

  1. Water quality: Maintain ammonia at 0 ppm, nitrite at 0 ppm, nitrate below 20 ppm. Perform 25–50 % weekly water changes with temperature-matched, dechlorinated water. Test weekly with a liquid test kit (not strips).
  2. Quarantine: Every new fish, plant, and piece of equipment should pass through a minimum 4-week quarantine period in a separate system. This single practice prevents more disease outbreaks than any medication. See our quarantine guide.
  3. Nutrition: Feed a varied diet including high-quality pellets, frozen foods (bloodworms, brine shrimp, mysis), and fresh vegetables where appropriate. Soak dry foods in a vitamin supplement (Seachem Garlic Guard + VitaChem) weekly.
  4. Stocking discipline: Avoid overcrowding. As a rough guide, allow 2.5 cm of adult fish length per 4 litres of actual water volume — and less for high-waste species like Oscars and Goldfish.
  5. Observation: Spend 5 minutes daily watching your fish. Changes in behaviour — loss of appetite, clamped fins, colour fading, unusual hiding — often precede visible disease symptoms by days. Early intervention is always more successful.

Can fish diseases spread to humans?
Most fish diseases cannot infect humans. However, Mycobacterium marinum (Fish Tuberculosis) can cause skin infections in people with open wounds who handle infected fish or contaminated water. Always wear gloves when working in tanks with sick fish and wash hands thoroughly afterward. People with compromised immune systems should take extra precautions.
Should I treat the whole tank or just the sick fish?
For highly contagious parasitic diseases like Ich and Velvet, treat the entire display tank — all fish are exposed even if only some show symptoms. For bacterial infections like fin rot, treating in a separate hospital tank is preferable to avoid disrupting the beneficial bacteria in the main tank filter. Always remove activated carbon before medicating, as carbon absorbs most medications.
How long should I quarantine new fish?
A minimum of 4 weeks is the standard quarantine period. This allows time for most diseases to manifest — Ich can appear within 3–7 days, but some diseases like Fish Tuberculosis may take weeks. During quarantine, observe closely for any symptoms and optionally run a prophylactic treatment (praziquantel for parasites, a mild antibacterial). Only transfer to the display tank if the fish has been eating well and showing no signs of disease for the full quarantine period.
Why do my fish keep getting sick despite treatment?
Recurring disease almost always points to an unresolved environmental problem rather than treatment failure. Test water parameters, check for temperature fluctuations (use a min/max thermometer), evaluate stocking levels, and review filtration capacity. If ammonia or nitrite is detectable, or nitrate regularly exceeds 40 ppm, no medication will produce lasting results. Address the root cause first, then treat the disease.
Can I use human antibiotics for fish?
While some human antibiotics (amoxicillin, erythromycin, metronidazole) contain the same active ingredients as fish medications, using human-grade pharmaceuticals is not recommended without veterinary guidance. Dosing is different for aquatic environments, inactive ingredients may be toxic to fish, and improper antibiotic use contributes to resistant bacterial strains. Use purpose-formulated aquarium medications from established brands like Seachem, API, or Hikari.
